Submitting a Counter-Notification
If you believe your content was removed by mistake or misidentification, and you have the right to use it, you may send us a counter-notification including:
Your physical or electronic signature (your full legal name).
Identification of the content that was removed and the location where it appeared before removal.
A statement, under penalty of perjury, that you believe the removal was a mistake or misidentification.
Your full name, address, telephone number, and email address, along with a statement that you consent to the jurisdiction of your local federal court and will accept service of process from the person who submitted the original DMCA notice.
